How to File Claim
- The proof of Claim Deadline to file claims has been temporarily deferred and will be determined at a later date. As such, claims are no longer required to be filed before Nov. 23, 2020.
- Proof of Claim forms are filed with court at the address below, not with PFI, PISF or counsel for the companies.
For your convenience, we are providing you with the Proof of Claim form you will need in order to file a claim against Professional Financial Investors, Inc. (“PFI”) and Professional Investors Security Fund, Inc. (“PISF,” and together with PFI, the “Debtors”) in the jointly-administered chapter 11 bankruptcies currently pending in the United States Bankruptcy Court for the Northern District of California, Case No. 20-30604 and Case No. 20-30579.
The Proof of Claim Form 410 along with instructions from the Bankruptcy Court on how to fill it out are attached here. You must use the Proof of Claim Form 410 to file a claim against either of the Debtors for money you believe you may be owed.
PLEASE NOTE: We are unable to answer any questions regarding how to fill out the Proof of Claim Form and cannot provide you with legal advice. If you require assistance or advice on how to fill out a Proof of Claim Form you should seek assistance from your own legal counsel.

Please do not send completed claims to PFI, PISF or their proposed counsel, Sheppard, Mullin, Richter & Hampton LLP (“SMRH”). The completed claim forms will only be processed if you submit them through one of the methods described above.
